Full-Time Benefits (32-40 hours scheduled workweek - available after 90 days employment):
BENEFIT REQUIREMENTS/ELIGIBILITY
HEALTH/DENTAL/VISION: 1st of month following 90 days
Pre-tax deduction
Shared cost with employer based on coverage levels
LIFE INSURANCE: Employee only
Paid by employer
Value of lesser of annual salary or $50,000
PENSION PLAN: 3 years employment
PAID HOLIDAYS: Six paid holidays per year
PAID TIME OFF (PTO): Begin accruing 1st day of employment
Use for vacation, sick and personal reasons
Accrual rate based on length of service
BEREAVEMENT LEAVE: 3 days (in state)
5 days (out of state)
For death of immediate family member
JURY DUTY: Paid difference in jury pay
Maximum 5 days per year
EDUCATION: Up to $2500 per year for approved courses

REPORT/CALL IN PAY: Two hour minimum pay for scheduled or call-in shift
